Work-at-Home Maternal Health Case Manager for EmblemHealth

EmblemHealth, the parent company of HIP Health Plan of New York and GHI, Group Health, Inc. is currently seeking a nurse to serve as a maternal health case manager for their Mom-To-Be Program in Melville, NY.  Pay is competitive, and an excellent benefits package is provided. You must possess a current, valid professional license in NY State (CCM or CSW desirable).  You must have 3 years of direct patient care in maternal child health. Obstetrical certification is preferred.

You also must have a strong utilization and managed care background; 2-3 years case management experience in managed care setting preferred. This position offers a 4-day condensed workweek and a telecommute option.
